Repeat Prescriptions and Forms

To organise repeat prescriptions, please phone between the hours of 2pm and 3pm, fax or e mail in advance and allow 24 hours before collection.

If a prescription from the hospital requires transfer on to a GMS prescription, please drop the script in to the front desk and we will have it ready on the next day.

Private prescriptions and forms may incur a charge.

Forms for completion by the doctors should be left in at reception.

Please allow at least 1 working day for completion on most forms.

If you are contacting the surgery for social welfare certificates, please do so between 4 pm and 5 pm.

More complex forms may require longer turn around time.

Driving license forms require attendance in person for an appointment for an eyesight test and review of medical history.
