Older Adults and Senior’s Health

GPs will have many dealings with older patients. It is important to remember that many people stay well and healthy and living in their own family home, independently, all their lives, or at least, to an advanced age.

Some people will need extra help from carers in their family and some will need help from external carers. Our family practice team have much experience with keeping people independent in their own home and sharing their medical care with Family or professional Home Carers.

Routine services can include assessments for memory disorders or cognitive impairment We offer selected Home GP Visits to the frail elderly We offer GP care or long term care medicine for patients in nursing homes Lab tests and diagnosis Chronic disease management Co-ordination of hospital care
